Our Clinic
Aviva Health, a nonprofit Federally Qualified Health Center with 11 locations in Douglas County, is the residency's continuity clinic.

Aviva Health is an independent, nonprofit Federally Qualified Health Center. It serves the people of Douglas County regardless of insurance or ability to pay, with a sliding fee scale based on family size and income. Its purpose is to reduce health disparities and improve access to care for underserved communities.
Aviva Health began as a small community clinic in 1991. It is now a network of 11 locations providing medical, dental and behavioral health services across Douglas County. In 2025, Aviva Health provided more than 72,000 patient visits. The State of Oregon recognizes it as a Patient Centered Primary Care Home.
For residents, this is a busy, community-focused practice with a diverse patient population. Care starts with listening to patients, understanding their lives and goals, and building long-term relationships. Residents work with a large multidisciplinary team: nurse case managers, primary care behavioral health clinicians, social determinants of health support staff, dietitians, a psychiatric nurse practitioner, and a developmental pediatrician. Specialized services at Aviva include pediatrics, dermatology, women's health, osteopathic manipulative treatment (OMT), and medication-assisted treatment (MAT) for substance use disorders. Residents learn whole-person care for patients who often face barriers to healthcare.
The clinic is next door to Costco, so errands and lunch fit between sessions.
