Medical Student Rotations
Third- and fourth-year rotations at Aviva Health, and sub-internships at Mercy Medical Center, by request to the residency office.

Medical students who want to rotate in Roseburg in their third or fourth year send a completed request form to the residency office at residency@aviva.health. We are not accepting international students for rotations at this time.
Download the Family Medicine Rotation Request Form.
The office arranges rotations with residency faculty at Aviva Health, our main family medicine site. We do our best with every request, but a request does not guarantee a place. Once your request is processed, the office emails you to say whether it was approved. If you have not heard in 3 to 4 weeks, email the office to follow up.

To fit as many students as we can, and to keep room for fourth-year sub-internships, we ask students to choose from these set blocks.

Fourth-year rotations: what to send
The request form
A current CV or resume
Unofficial transcripts
Unofficial COMLEX or USMLE transcripts
A professional headshot, for your Aviva ID badge
A short paragraph: what interests you most about our residency program?
Send everything as one packet in one email. Incomplete requests wait until every item has arrived.
Requirements for every student at Aviva Health
A current affiliation agreement (or MOU/MOA) between your medical school and Aviva Health must be in place before your rotation starts.
Fourth-year students who plan to apply to the residency are assigned a sub-internship: two weeks on the inpatient service at Mercy Medical Center and two weeks in the outpatient clinic. For that, your school needs a current affiliation agreement with both Aviva Health and Mercy Medical Center.
Send all clearance documents to Aviva Health Human Resources at least 60 days before your start date:
A letter of good standing from your medical school
A copy of your school's malpractice and liability insurance
Your current CV or resume
A copy of your ACLS and BLS cards
Your immunization records, including hepatitis B and a negative TB test within the last year
Students attend orientation at Aviva Health before or on their first day and receive an Aviva Health ID badge. Wear it at all times in the clinic.
Questions about affiliation agreements, clearance documents or orientation go to the residency office at residency@aviva.health.
If you will also rotate at Mercy Medical Center
Your school must have an affiliation agreement with Mercy Medical Center for the inpatient service. Start that process at least 60 to 90 days before your rotation, so the agreement can be signed by both your school and the hospital. Questions about Mercy affiliation agreements go to Evelyn Klemmer at evelyn.klemmer@commonspirit.org or 541-677-2368.
Once your rotation is approved, send the required clearance documents to the Mercy Medical Center Medical Staff Office at least 45 days before your start date. Melissa Ragon, Director of Medical Staff Affairs, has the list: melissa.ragon@commonspirit.org or 541-677-4373.
Observerships and shadowing
CHI policy does not allow observerships or shadowing of any kind at CHI Mercy Health Mercy Medical Center or its affiliated clinic sites. Umpqua Community Health Center (dba Aviva Health) does not allow them at any clinic site either. See the Policy for Observership and Shadowing.
